Confirm your account still hosts the required data; data may be deleted after a period of inactivity.

Recognize that no phone/email/chat support is included with temporary access or license.

If approved, your data export files will be sent to the admin requestor on the form or your account will be reactivated for 24 hours (temporary license) as specified.
After the time period ends, access will be deactivated.
